Problem Statement :

Create a form which is intriguing and should generate a higher rate of completion.

UX Solution :

We created a story format to engage the audience so that the form is relatable for them. I created illustrations and an interactive rating slider to elevate the experience.

UX Solution

We created a story format so that users can relate to the story with the help of expressive illustrations.
Each rating had an interactive emoticon that became visible every time a user clicked on a number. This made the survey interesting and intruguing and in turn generated more form completion.

Illustrations

The illustrations were an integral part of the survey form and needed to be as detailed as possible. The expressions needed to be visible and the form needed to be easy to understand.
